エクスチェンジ (保険市場)の創設 - オバマケアの1つの特徴

exchangeという言葉が、オバマケアをめぐる報道のなかで盛んに出てくる。探すと次のようなかなり懇切丁寧な説明が出てきた。アメリカ人にとっては、オバマケアがどう具体化されて実行に移されるのかは、重大な日常的関心事である。自分たちがどのような健康保険に加入できるのか、その費用負担はできるのか、という切実な問題だからである。下記の情報もそうした国民にたいし、説明しているものといえる。
 exchange というのは、簡単に言うと、保険業者が保険の提案を出し(その提案内容は連邦政府の設定する基準を満たすものでなければならない。たとえば既往症のある人を排除することはできないなど)、国民や企業者がそれを比較検討して購入する -そういう市場のことである。交換する場という意味で用いられている。Health Insurance Exchange (HIX)ともいわれる。
 これはオンラインの市場である。つまりウェブにアクセスして必要事項を記入し、関心のある保険商品を購入するとしたら、どれくらいの金額が必要なのか、そして自分の所得や雇用状況からどのくらいの補助金がもらえるのか、といったことがオンラインで計算し、加入できる仕組みになっている(まだ稼動していない)。
 exchange には3種類がある。1つは純粋に州レベルで運営するもの、1つは連邦政府が代わって運営するもの、もう1つはその中間である。
 exchangeに参加できる資格にはいろいろと制限があって、なかなか複雑である。
 アメリカには国民皆保険に批判的な人は想像もできないくらい多い。しかし、3000-4000万人が未保険者であるという現実に目を向けないという状況は、それ自体、大いなる問題であろう。

The ObamaCare Health Insurance Exchange (HIX) opens Oct 1st, 2013. The ObamaCare health insurance exchanges, or ObamaCare exchanges, are State or Federal run online marketplaces. Americans can use the "Affordable" Insurance Exchange exchange to buy health insurance from private health care providers. Shoppers can use a price calculator to find the best deal for them and their family. The health insurance exchange is estimated to provide up to 23 million people with affordable health insurance.

"ObamaCare's" State Health Insurance Exchanges: States can build a health insurance exchange (HIX) on their own, partner with one or more other states, or have the federal government build and run the insurance exchange for them.
Understanding the Exchange Semantics: The HIX, ObamaCare Exchanges, Health Benefit Exchanges, Health Insurance Marketplace, Health Insurance Exchanges and Affordable Insurance Exchanges are all the same thing. There is no official term yet. Just like ObamaCare is also called health care reform, the Affordable Care Act or ACA, etc.
ObamaCare Exchanges: State Insurance Exchange Time Line
The states had until November 16th to submit a declaration letter of how they will handle their state health insurance exchange.
States who were going to run their own "ObamaCare" exchanges had until December 14th 2012 to submit an initial Blueprint application. The HHS approved the states exchange on Jan 1st, 2013.
The final deadline for both the Declaration Letter and Blueprint Application for State Partnership Exchanges was extended to Friday, February 15, 2013. How Will The ObamaCare State Run Health Insurance Exchanges and Federal Run Exchanges Work?
The ObamaCare health insurance exchanges allow all Americans above the poverty line to be able to buy health insurance through providers (who meet the standards of ObamaCare) via an online market place (similar to how buying online car insurance works now).
Each health care provider will try to attract customers by providing affordable quality health care packages.

What Type of Plans Are Available on the ObamaCare Exchange?: Bronze, Silver, Gold and Platinum
There are three tiers of plans you or your employer can purchase on the exchange they range from lower quality, but more affordable "Bronze plans", to "Silver plans" to a more expensive plan with better coverage called a "Gold plan". There is also a "Platinum plan" which is the highest quality and cost plan. Lower premium plans will have higher deductibles.
